Sports Physical Requirements
 
In order for any student to try out for any after school sponsored sport, a Delaware Interscholastic Athletic Association, (DIAA) form must be completed. This is a 4 page form that is completed by a parent/guardian, the student and a physician. In order for your child to be eligible to participate in sports for the 2012-2013 academic year, the physical exam needed to be completed on or after (more recent than) April 1, 2011. One completed DIAA form is good for all sports for this entire academic year. The form can be downloaded by clicking the link at the bottom of the page.

Sports physicals are available at the Naamans and Silverside Roads walk in clinics. Call ahead regarding availability and fee. As always, you may have the exam performed by your own physician.
